How does Linatab E 5/10mg Tablet work?

Empagliflozin and linagliptin both act through different mechanisms to improve glycemic control in patients with type 2 diabetes mellitus:

Empagliflozin inhibits the sodium-glucose co-transporter 2 (SGLT2) in the kidneys. SGLT2 is responsible for reabsorbing glucose from the glomerular filtrate back into the bloodstream. By inhibiting SGLT2, empagliflozin reduces the reabsorption of filtered glucose by the kidneys, thereby increasing the amount of glucose excreted in the urine. This leads to a decrease in blood glucose levels.

Linagliptin is an inhibitor of dipeptidyl peptidase-4 (DPP-4), an enzyme that degrades incretin hormones such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). By inhibiting DPP-4, linagliptin increases the concentration of active incretin hormones, which stimulate the release of insulin from pancreatic beta cells in a glucose-dependent manner. Additionally, GLP-1 inhibits glucagon secretion from pancreatic alpha cells, thereby reducing hepatic glucose output.

Overdose Effects of Linatab E 5/10mg Tablet

In the event of an overdose, standard supportive measures should be employed. These may include removing unabsorbed material from the gastrointestinal tract, clinical monitoring, and implementing appropriate clinical interventions if necessary.

Disease Interactions:

Contraindications

Hypersensitivity to the active substance or any of the excipients.

Pregnancy & Lactation

• The use of Linagliptin has not been studied in pregnant women. It's advisable to avoid using Linagliptin during pregnancy unless the potential benefits outweigh the risks.
• The risk to breastfed infants cannot be ruled out. A decision should be made whether to discontinue breastfeeding or discontinue Linagliptin therapy, taking into account the benefits of breastfeeding for the child and the benefits of therapy for the woman.

Precaution & Warnings:

• There have been reports of acute pancreatitis, including fatal cases, associated with Linagliptin. If pancreatitis is suspected, discontinue Linagliptin promptly.
• Assess and correct volume status before initiating Linagliptin in patients with renal impairment, the elderly, those with low systolic blood pressure, and those on diuretics. Monitor for signs and symptoms of hypotension during therapy.
• Monitor renal function regularly during Linagliptin therapy, especially in patients with an estimated glomerular filtration rate (eGFR) below 60 mL/min/1.73 m2.
• Consider reducing the dose of insulin secretagogues or insulin to minimize the risk of hypoglycemia when initiating Linagliptin therapy.
• Monitor for genital mycotic infections and treat them as appropriate.
• Monitor for urinary tract infections and treat as appropriate.
• Discontinue Linagliptin promptly if hypersensitivity reactions occur. Assess for other potential causes, initiate appropriate monitoring and treatment, and consider alternative diabetes treatments.
• Monitor and treat elevated LDL-cholesterol levels as appropriate during Linagliptin therapy.
• There's no conclusive evidence from clinical studies demonstrating macrovascular risk reduction with Linagliptin or any other antidiabetic drug.
